In Eq.2 age is included as a dummy variable (dummies are added to the age groups), while all other variables are the same as in Eq.1. In Eq.3, education is included as a dummy variable (dummies are added to the educational levels), while all other variables are the same as in Eq.1. In Eq.4, the frequency of health services usage is included as a dummy variable (dummies are added to the categories of usage frequency), while all other variables are the same as in Eq.1
